- Roger Kwok will continue to implement the reformation of the health care system. He will propose a new plan for medical reform. His approach and beliefs are vastly different from the newly joined Nancy Wu; thus, the two confront each other for their own ideas. He, Kelly Cheung, and Moses Chan have a triangular relationship. Moses joins the hospital becauses he wants to woo his ex, Kelly, again; however, Kelly has her heart for Roger.
- Kenneth Ma will play the assistant administrative director and the head of cardiothoracic surgery. The plot will explain the reason for the problems in his relationship with Natalie Tong.
- Moses will play Vincent Lee Man Sun (李文信), who was originally a doctor without borders. When he returns to Hong Kong, he meets his ex, Kelly, again; because he wants to woo her again, he goes to work at Maxwell Paxton Hospital, becoming an associate consultant in the Accident & Emergency (A&E) department. He has an unreserved, handsome appearance, so he is surrounded by many females; thus, Kelly feels no sense of security. He, Kelly, and Roger have a complicated romantic entanglement. His scenes add lightheartedness to the main storyline so that the plot is not entirely heavy.
- Nancy will play Ip Ching (葉晴), the administrative director and the doctor in charge of the oncology department. She is a cool and rather tough doctor with attitude, and she is a go-getter. She has many scenes with Kenneth, but they are not a couple. She will get involved in Kenneth and Natalie's relationship, having an ambiguous relationship with Kenneth; however, her character is simply an ornament in their relationship. There is some underplot in her relationship with Kenneth, but Kenneth and Natalie's relationship is already very stable. Her first line in the series is "to eradicate Maxwell Paxton Hospital and turn it into history". She also has many scenes with Roger; they are on opposing sides. In principle, there are no bad people. Her appearance is like completely overturning everything Roger did in the previous installment. Although they have different positions, they also want to make medical reforms.
- Natalie and Kenneth have a rocky romance. She and Kenneth really love each other, but they have a knot in their hearts that cannot be unravelled. Kenneth is a very attractive doctor, so there will be a female (Nancy) who likes him no matter what, but it is not a problem between them, so it is not considered a triangular relationship.
- Kelly, Moses, and Roger have a sort of a triangular relationship.
- Virginia Lau will play Cheung Ting (張庭), the director of TNN's news department. She has a strong personality. She does not smile much, is goal-oriented, does things by fair means or foul to achieve her goal, and has her own strong views. She covers the sky with one hand, as she controls all of Hong Kong's media. She has a strong sense of jealousy, and she goes around different men. She is Roger's ex-wife. She returns to Hong Kong and is in opposition with Roger on medical reform and a lot of things. In the end, she and Moses have a one-night stand.
- Shaopin Tsui will play Yuen Chan Fai (阮振輝), a resident physician in cardiothoracic surgery.
- Lesley Chiang will play Rachel Fan, a resident physician in cardiothoracic surgery.
- Patrick Lam will play Ben Ling Wai Hong (凌偉康), an associate consultant in neurosurgery. He is Roger's right-hand man. He has the most scenes with Roger and Nancy. He tries to gain Roger's favour.
- Fei Wong will play Wendy Tsui Hiu Wan (徐曉韻), a resident physician in oncology.
- Venus Tang will play Kami Lam Ka Mei (林嘉美), a houseman.
- Henry Lee will play Tak Hin (德軒), a houseman in cardiothoracic surgery.
- Irina Tang will play Liu Hoi Yan (廖凱欣), a houseman in cardiothoracic surgery.
- Nicole Wu will play Lau Kei Kei (劉琪琪), a registered nurse.
- Amy Ng will play an anchor.
- Linus Ma will play Ma Chun Lok (馬俊樂), a houseman in cardiothoracic surgery.
- Keefe Ng will play Au Chi Wing (歐子穎).
- Joan Lee will play Cheung Ka Po (張家寶).
- Rosita Kwok will play a patient with panic disorder. She is Moses' patient. She has a romantic storyline with Moses.
- Wiyona Yeung will play Amanda.
- Aliya Fan will play Chan Chi Fei (陳芷菲), a registered nurse in the A&E department.
- Jarryd Tam will play Lee Hiu (李曉).
- Carmen Ngai will play a nurse.
