- Kevin Cheng will play Jonathan Hui Kam Fung (許甘楓), a paediatrician. He and Kenneth Ma were originally good friends, as they went to high school and studied medicine together; however, because of a matter, they severed their relationship. He feels pained that Kenneth has become like this, but he has always considered him his friend in his heart. He is also called 「鬍鬚醫生」 and 「阿焚」.
- Kenneth will play Amos Fong Chung Yan (況叢昕), a paediatric surgeon. He is righteous, but he would cross the boundaries, as he feels that there are a lot of ways to help people. He always does under-the-table dealings. He is quite ambitious, as he wants Hong Kong to set up a paediatric surgery centre, but there are many difficulties that are hindering him. He had some experiences, so it feels as though you do not know whether he is a good or bad person. He and Catherine Chau are friends but not yet lovers.
- Linda Chung will play Eman Cheung Yi Sam (章以芯), a doctor without borders who later returns to work at a city life hospital. She has medical ethics and brilliant medical skills. She is a cardiac surgeon. She and Kevin always make snide remarks at each other.
- Catherine will play Kay Mak Hoi Kei (麥海琪), a surgeon.
- Him Law will play Max Man Pak Hei (文柏熙), who is like a crown prince. His mother, Mimi Kung, has the highest position in the series; thus, in the beginning, he is afraid of people finding out his identity because he is scared that people would be prejudiced toward him. His mother was too outstanding, so he has a lot of pressure, and he took extreme measures before, and then later returns to being a doctor. He has an illness; it is as though he can treat others but cannot treat himself.
- Mimi will play 「鍾教授」, a professor of medicine. She is a steady person but is also sensitive to people's needs and is full of smiles. She is powerful on the inside and is very competent.
- Florence Kwok will play Joey Kwok Bo Ching (郭葆晶), a senior doctor in the department of anesthesiology.
- Gabriel Harrison will play Paul Chow Chan Yiu (周振堯), the CEO of Princess Anne Hospital (安妮醫院). He is slightly annoying but adorable. He has the most scenes with Kenneth.
- Regina Ho will play Yau Chi Wa (游芝樺), a houseman. She is a gentle and kind person.
- Bowie Cheung will play Candice Lin Cheuk Ying (連卓盈), an anesthesiologist. She is Florence's pupil. She and Him are a couple.
- Arnold Kwok will play Tang Wai Chung (鄧偉總), a paediatrician.
- King Lam will play a paediatrician.
- Jan Tse will play Dr. Lui.
- Andy Lin will play Chris Yiu Pok Yan (姚博仁), Linda's pupil.
- Niklas Lam will play Yim Ka Ming (嚴家明), a paediatric houseman. Linda is his superior.
- Linna Huynh will play Hayley Lee Wing Huen (李穎萱), a medical intern. She has scenes with Kenneth.
- Kelly Ng will play Bobo Ng Lai Po (伍勵寶), a registered nurse at Princess Anne Hospital. Him is her elder cousin.
- Randal Tsang will play a doctor.
- Bonnie Lui will play the younger version of Linda's character.
- Fei Wong will play Wan Chi Tung (溫紫彤), a nurse. She has the most scenes with Kevin and Linda.
- Yuki Law will play a nurse.
- Rachel Ching will play Wong Ching Wan (王靜韻), who works in the PICU.
- Mina Kwok will play Wong Mei Chan (黃美珍), a nurse.
- Kanice Lau will play To Yik Lai (杜奕禮), who works in the PICU.

FYI: The Chinese title of this series was previously 《兒童醫院》 (literally translated as "Children's Hospital"), and has been changed to 《星空下的仁醫》 (literally translated as "Doctors Under the Stars"). The official English title is "Kids' Lives Matter".
